1:30 p.m. The Importance of Model Adherence & Fidelity

Table Discussions: 
Fidelity Prevention Coordinators will review Logic Models, Fidelity Processes and Tools, and discuss the Quality Assurance Process.  Providers will have an opportunity to ask questions and share ways that they plan to or have used the fidelity monitoring process to improve quality.
2:15 p.m. Data Collection and Reporting
  • Lee Ann Cook Outcome measurement tools/processes, quarterly spreadsheets, data roll-up and review process, community report templates
  • PCCD Representative E-grants – putting reports in Grant Changes – PMR, Budget Modification
Table Discussion: Collecting Data and Demonstrating Outcomes Prevention Coordinators will provide examples of data collection tools, explain the spreadsheets, and discuss tool alignment with the logic model and intended outcomes.  Providers will be asked to discuss ways that they plan to or currently use data in their work.
